By signing below, I acknowledge that training at 3D Martial Arts involves physical activity and carries risks of injury or illness. I voluntarily accept all such risks and release 3D Martial Arts, Brandon Thatch, their staff, and affiliates from any liability for injury, damage, or loss, including those caused by negligence.
I confirm I am physically able to participate and take full responsibility for disclosing any health conditions. I understand no medical clearance is required and I am training at my own risk.
For Minors: As a parent/guardian of a participant under 18, I give permission for my child to participate and agree to the terms above on their behalf.
Media Release: I allow 3D Martial Arts to use photos or videos of me or my child for promotional purposes. Last names of minors will not be published.
No Refund Policy: I understand all memberships are non-refundable and auto-renew monthly unless canceled before the next billing date.
